You can report a work accident or an occupational disease without signing in. Choose what you are reporting below. If you are an employer, doctor or occupier reporting on behalf, please sign in to e-Services to lodge the report.
Work accident (AC)
Report an injury from an accident at work — as the injured person, their representative or their next-of-kin.
Occupational disease (OD)
Report a work-related illness or disease — as the affected person, their representative or their next-of-kin.
Already submitted a report? Retrieve and view it using the password shown on your acknowledgement.
Reports are lodged with MOM for review. Provide accurate contact details so we can follow up if needed.
